Registered Nurse – Community Older Persons Mental Health

About the role

The South Eastern Sydney Local Health District is seeking an experienced Registered Nurse to join its Older Persons Community Mental Health Team. You will provide recovery‑oriented, person‑centred care to adults aged 65+ living with mental illness in community settings.

Key responsibilities

• Conduct comprehensive mental health assessments and develop individualized care plans.
• Coordinate care with families, aged‑care providers, GPs and other health professionals.
• Deliver evidence‑based therapeutic interventions across a range of community locations.
• Promote independence, wellbeing and quality of life for older consumers.

Required profile

• Current unrestricted AHPRA registration as a Registered Nurse.
• Demonstrated experience assessing and managing older people with mental illness in a community setting.
• Commitment to a consumer‑focused, recovery‑oriented approach.
• Effective communication and teamwork skills.

Required skills

• Proficiency with electronic medical record (EMR) systems.
• Computer literacy and basic IT skills.
• Valid driver’s licence for community visits.
