Senior Community Services Pharmacist

About the role

Join Metro South Health’s Community and Oral Health Services Pharmacy team as a Senior Community Services Pharmacist. You will deliver evidence‑based, patient‑centred pharmacy services across home visits, outpatient clinics, residential care and community settings.

Key responsibilities

• Provide comprehensive clinical pharmacy services including medication assessment, intervention, review and discharge planning.
• Collaborate with multidisciplinary teams, community pharmacies, general practitioners and other healthcare providers to ensure continuity of care.
• Contribute to the development of clinical practices, protocols and service‑improvement initiatives.
• Offer clinical advice, education, supervision and support to junior clinicians, students and support staff.

Required profile

• Experienced pharmacist with strong clinical expertise in community and home‑based settings.
• Excellent communication, interpersonal and collaboration skills.
• Strong clinical judgement, time‑management and ability to handle complex patient care.
• Demonstrated leadership, supervision experience and initiative in service improvement.
