About the role

Key responsibilities

• Conduct home environmental assessments to reduce fall risk and recommend assistive technology.
• Design and implement evidence‑based reablement programs that restore daily activities such as cooking, gardening, and personal care.
• Develop tailored exercise and gait plans to manage pain, improve strength, balance, and community mobility.
• Create cognitive support strategies and introduce aids for clients with dementia or cognitive changes.
• Educate clients, families, and carers on safe manual handling, energy conservation, and therapeutic techniques.

Required profile

• Qualified Occupational Therapist or Physiotherapist registered to practice in Australia.
• Passion for delivering hands‑on clinical care in the client’s home environment.
• Commitment to evidence‑based practice and client‑centred goal setting.
• Ability to work autonomously with a manageable caseload (5‑6 visits per day).

Required skills

• Clinical assessment and reablement planning.
• Knowledge of assistive technology and home modification principles.
• Exercise prescription and gait training techniques.
