Care Coordinator

About the role

Southern Cross Care is hiring a Care Coordinator in Klemzig, South Australia, for permanent full-time employment at 76 hours per fortnight on a three-week rotating roster covering AM, PM, weekends and public holidays. The Care Coordinator leads, coaches and mentors nursing and personal care staff, ensures care is holistic, person-centred and aligned with the "Better for Life" model, supports the Care Manager in daily operations and staff management, and applies clinical judgement to keep service plans up to date.

Key responsibilities

• Monitor quality assurance systems, conduct resident assessments, ensure timely medication administration and maintain accurate care plans.
• Act as an advocate for residents, respecting their goals and preferences, managing risks and tailoring care to their social, cultural and medical needs.
• Coordinate staff onboarding, give performance feedback, facilitate appraisals and monitor compliance with policies and procedures.
• Implement workplace health and safety policies, manage hazards and ensure staff follow safe work instructions.

Required profile

• Bachelor of Nursing with current unrestricted AHPRA registration as a Registered Nurse.
• Current First Aid Certificate (HLTAID011) and CPR Certificate (HLTAID009).
• Previous experience as a Registered Nurse with leadership responsibilities in an aged care setting.
• Commitment to the organisation's values of Service, Courage and Compassion and the ability to build positive relationships.

Required skills

• Proficiency in Google Workspace and healthcare software.
• Strong clinical management, time management and problem-solving skills.
• Deep knowledge of Aged Care Standards, dementia care and AN-ACC funding.
