Clinical Team Leader - NDIS Supported Independent Living

About the role

The client is an established provider of nursing, disability and Supported Independent Living services, delivering person-centred care focused on independence, dignity, safety and wellbeing. This full-time leadership role provides clinical and operational leadership across NDIS Supported Independent Living homes and related community supports, with flexible hours.

Key responsibilities

• Lead day-to-day operations of Supported Independent Living homes
• Supervise, coach and mentor Support Workers, and manage rosters and shift coverage
• Conduct assessments and maintain care plans, support plans and nursing assessments
• Oversee medication management and identify and escalate clinical risks
• Coordinate health appointments and support participant routines and personal goals
• Maintain compliance, documentation and stakeholder communication

Required profile

• AHPRA registration as a Registered Nurse
• Minimum 5 years' nursing experience
• Previous clinical, operational or team leadership experience
• First Aid, CPR certification and National Police Clearance
• NDIS Worker Screening Check or willingness to obtain one, and completion of the NDIS Worker Orientation Module
• Valid WA driver's licence and reliable vehicle

• Strong knowledge of duty of care, clinical risk and safe practice
• Excellent written, verbal and stakeholder communication
• Documentation, reporting and attention to detail
• Microsoft Office and relevant care and reporting systems
• Nice to have: experience in NDIS SIL, QUASR, ShiftCare, Employment Hero or SharePoint, medication governance, audit and investigation support, and the NDIS Practice Standards and Code of Conduct

What we offer

• Salary of $95,000 - $105,000 plus superannuation, under the Nurses Award
