About the role
As a Clinical Aged Care Assessor you will conduct comprehensive assessments for older adults, linking them to services that support independent living and high quality of life. The role is permanent part‑time, working four days a week with daily travel across the service area.
Key responsibilities
- Conduct high‑quality Aged Care assessments to identify client needs.
- Facilitate referrals to essential services and promote functional independence.
- Engage with families, carers and multidisciplinary teams as key stakeholders.
- Ensure compliance with Department of Health contractual requirements.
Required profile
- Minimum 1 year experience working with older Australians in assessment or service delivery.
- Tertiary health‑related qualification and professional registration (e.g., AHPRA).
- Current Australian driver’s licence, insured vehicle and ability to travel within the area.
Required skills
- Proficiency in Microsoft Word, Excel and Outlook.
- Experience using client management systems.
