Team Leader – Hospital to Aged Care Dementia Support

About the role

This is a leadership position within HammondCare’s Hospital to Aged Care Dementia Support Program (HACDSP) in partnership with Dementia Support Australia. The role is based in the Malvern office and focuses on managing a skilled team to deliver high‑quality dementia care services.

Key responsibilities

• Provide professional and clinical leadership, ensuring timely allocation of referrals and compliance with DSA KPIs.
• Oversee office safety, equipment maintenance, fleet vehicle usage, and ICT resources.
• Maintain stakeholder relationships, report performance to the Regional Manager, and handle customer complaints.
• Recruit and develop team members, manage budgets, and support continuous improvement of the DSA service.

Required profile

• Tertiary qualification in Nursing, Allied Health or a related health degree.
• Experience supporting older people and carers living with dementia, emphasizing psycho‑social case management.
• Ability to lead and support a multi‑site team of professionals.
• Current Australian driver’s licence and willingness to travel intra‑ and interstate.
