Team Leader – Dementia Support Program

About the role

This is an exciting opportunity to lead a skilled team delivering the Hospital to Aged Care Dementia Support Program (HACDSP) for Dementia Support Australia. You will provide professional guidance, foster a supportive culture, and ensure high‑quality service delivery for people transitioning from hospital to home or residential care.

Key responsibilities

• Provide clinical and professional leadership for team members, allocating referrals in line with DSA KPIs and the Activity Work Plan.
• Oversee office safety, routine maintenance, ICT equipment, and local fleet vehicle utilisation and reporting.
• Maintain stakeholder relationships, report performance to the Regional Manager, and manage customer complaints or challenging situations.
• Recruit and develop team members with appropriate skills, and ensure compliance with HammondCare policies and financial delegations.

Required profile

• Tertiary qualification in Nursing, Allied Health or a related health degree.
• Demonstrated experience supporting older people and carers living with dementia, using psycho‑social case‑management approaches.
• Ability to manage and support a multi‑site team of skilled professionals.
• Current Australian driver’s licence and willingness to travel intra‑ and interstate.
