About the role

The Northern Sydney Local Health District is seeking a dedicated Registered Nurse to provide community nursing care in patients' homes or clinics. This permanent full‑time position offers the chance to work within a supportive, inclusive team at the Hornsby Centre.

Key responsibilities

Assess, plan, implement and evaluate community nursing care for patients in their homes or clinic settings.
Collaborate with multidisciplinary teams to achieve quality patient outcomes.

Required profile

Registered Nurse – Division 1 with AHPRA registration.
Current NSW driver’s licence.
Demonstrated clinical knowledge and skills in safe, evidence‑based, person‑centred nursing care.
Effective communication and interpersonal skills for independent and collaborative work.
Strong organisational and time‑management abilities to manage a diverse caseload.

Required skills

Clinical assessment and care planning.
Clinical decision‑making and management of complex patient needs.
