Practice Manager - Care Coordination (NDIS & Aged Care)

About the role

Proveda, a community-led organisation serving older people, people with dementia, people with disabilities and others with complex health needs in Northern Sydney, is looking for a Practice Manager. You will lead and support the Care Coordination teams across Aged Care and NDIS services, providing leadership, coaching and professional supervision while driving best practice, service consistency and positive customer outcomes.

Key responsibilities

• Lead and develop Care Coordination teams
• Promote high-quality, goal-oriented service delivery
• Monitor service quality, productivity, customer outcomes and performance
• Support recruitment, onboarding and employee development
• Drive continuous improvement and change initiatives
• Manage complaints, incidents and quality activities

Required profile

• Tertiary qualification in Social Work, Social Sciences, Community Services or a related field
• Leadership experience in aged care, disability, community services, health or a related sector
• Experience coaching, mentoring and developing employees
• Current NDIS worker screening check and WWCC (or the ability to obtain them)

Required skills

• Strong knowledge of the NDIS and contemporary service delivery practices
• Knowledge of NDIS and Aged Care services and compliance requirements
• Strong communication, problem-solving and stakeholder engagement skills
