Health and Wellness Coordinator

About the role

The Health and Wellness Coordinator supports residents of Cobbitty Village to maintain their health, wellbeing and independence. Working onsite on Mondays and Wednesdays, you will act as a bridge between residents, families, healthcare professionals and community services.

Key responsibilities

• Assist residents with navigating My Aged Care, accessing health and home‑care services, and completing assessments.
• Coordinate referrals to allied health providers, community organisations and support services.
• Develop and maintain individualized Health & Wellness Plans for each resident.
• Organise and promote resident education sessions, wellness programmes and social activities.
• Maintain accurate records of assessments, referrals and service outcomes.
• Collaborate with Village Managers, Care Operations Managers and external stakeholders to ensure timely support.

Required profile

• Demonstrated empathy and professionalism when working with older adults.
• Strong communication and relationship‑building skills with residents, families and service providers.
• Ability to work independently and as part of a collaborative team.
• Commitment to resident‑centred care and continuous improvement.
