About the role

Join Dementia Support Australia's Staying at Home Program to improve the lives of people living with dementia and their carers. The role involves client screenings, assessments, facilitating carer sessions, and contributing to program improvement.

Key responsibilities

• Assess client suitability for the Staying at Home program via phone, Telehealth, or onsite assessments.
• Deliver services in line with psychosocial case management principles using phone, Telehealth, face‑to‑face or email.
• Maintain accurate client records in the CRM system and ensure data confidentiality.
• Collaborate with medical specialists and contribute to quality improvement initiatives.

Required profile

• Experience in Australian health, hospitals or aged care (community or residential).
• Knowledge of dementia types, presentations and behaviour management strategies.
• Current Australian driver’s licence and willingness to travel interstate, including regional and overnight trips.
• Qualifications in Allied Health, Nursing, Aged Care, Dementia or Mental Health (highly desirable).

Required skills

• Proficiency with Telehealth platforms.
• Experience using CRM or online client management systems.
