Clinical Partner – Occupational Therapy (Part‑time)

About the role

HammondCare is seeking a part‑time Clinical Partner – Occupational Therapist to support clients across the Illawarra region, based in Nowra or Horsley, NSW. You will join a multidisciplinary Clinical Care & Practice Team delivering person‑centred, relationship‑based care in homes and community settings.

Key responsibilities

• Assess functional care needs of older adults and people with complex health conditions.
• Develop and implement individualized treatment plans, set clear care goals, and provide interventions.
• Conduct client home visits and facilitate group activities in both in‑home and clinic‑based environments.
• Collaborate with a multi‑disciplinary team to ensure continuous quality improvement and compliance with AHPRA standards.

Required profile

• Tertiary qualified Occupational Therapist with current AHPRA registration.
• Practical experience in rehabilitation or aged‑care settings (inpatient, outpatient, or community).
• Strong communication skills and ability to manage time effectively.
• Access to a reliable vehicle, valid driver’s licence and safe driving record (travel costs reimbursed).
