Case Manager - Homelessness Support, Mount Isa

About the role

Centacare North Queensland, a not-for-profit providing counselling, family support, disability services, NDIS services and more across North Queensland, is hiring a Case Manager in Mount Isa. The position delivers an assertive outreach model to individuals and families who are homeless or at risk of becoming homeless, providing information, referral, advocacy support and links to services so people can access secure housing options.

Key responsibilities

• Provide case management support appropriate to the program target group and develop early intervention strategies to engage participants and meet program and individual goals.
• Carry out intake, triage and initial assessment to determine risk, urgency and eligibility, prioritising high-risk or crisis presentations.
• Use electronic data systems to record participant information in line with service agreements, legislation and program policies.
• Build community partnerships through networking, referrals and promotion of Centacare NQ programs.
• Take part in meetings, professional supervision and professional development.
• Carry out outreach and community engagement activities as required.
• Help plan and deliver community education and support, and comply with workplace health and safety, privacy, equal employment opportunity and quality management requirements.

Required profile

• Degree in Social Work, Human Services or Behavioural Sciences, or a Level 4+ community sector qualification plus at least two years' case management experience, including culturally appropriate community work, assessment and referral.
• Current driver's licence.
• Disability Worker Screening and Working with Children Blue Card, or eligibility to obtain and maintain them.
• Desirable: group facilitation experience, large multi-disciplinary team experience, willingness to work outside normal office hours under the SCHADS Award, and experience in social/community services or not-for-profit organisations.

What we offer

• Salary from $92,286 depending on experience and skills, plus a $2,500 per year above-award allowance and a $5,000 Mount Isa location allowance.
