Cardiac Educator – RN/AHP (Remote Aboriginal Communities)

About the role

The Cardiac Educator will join Sunrise Health Services Aboriginal Corporation’s Chronic Care team in Katherine. Working across remote Aboriginal communities, you will provide culturally safe cardiac education, clinical support and health‑promotion activities to improve heart health outcomes.

Key responsibilities

• Deliver individual and group cardiac education sessions that are culturally appropriate and support self‑management.
• Facilitate health‑promotion workshops focused on heart health, rehabilitation and chronic disease prevention.
• Collaborate with multidisciplinary teams and external partners to plan and implement evidence‑based cardiac care.
• Provide clinical leadership by mentoring health professionals and developing locally relevant educational resources.
• Collect and report data, review care plans and ensure adherence to clinical guidelines and quality‑improvement processes.
• Travel regularly to remote communities using company vehicles or charter flights.

Required profile

• Tertiary qualification in Nursing or Aboriginal Health Practice with current AHPRA registration.
• Minimum three years of experience in cardiac care delivering patient‑centred services.
• Demonstrated understanding of Aboriginal health perspectives and ability to work effectively in community settings.
• Current unrestricted driver’s licence (manual preferred) and willingness to travel to remote locations.
• Current First Aid certification, acceptable vaccination status, National Criminal History Check and NT Ochre clearance.
