Stepped Care Coordinator / Care Coordination Specialist

About the role

Based in our Gympie office, the Stepped Care Coordinator is the first point of contact for clients referred through the Head to Health Phone Line and other pathways. You will guide clients through a smooth entry into our services and support their journey to well‑being.

Key responsibilities

• Provide direct engagement and brief interventions to ensure smooth transitions and after‑care.
• Collaborate with internal teams and external partners to deliver comprehensive, person‑centred care.
• Oversee collaborative care planning, service transitions and after‑care processes.
• Build and maintain relationships with local hub partners and the lived‑experience workforce.

Required profile

• Extensive experience in mental health and/or AOD settings, including knowledge of systems, legislation and brief‑intervention approaches.
• Ability to work independently and as part of a community‑based recovery team.
• Qualifications in psychology, social work, mental health nursing or a related field; relevant registration or professional membership preferred.
• Current state‑based driver’s licence.
