Service Coordinator - Aged Care (Part-Time)

About the role

The Junction Neighbourhood Centre (JNC) is a community-focused organisation in the Inner City and Eastern Suburbs of Sydney, working within the Commonwealth Home Support Programme (CHSP) to give older people social support and respite so they stay active, independent and connected. This part-time role (28 hours per week, Tuesday to Friday) sits at the heart of a busy aged services team, combining people leadership, service coordination and client engagement. It is based at the Glebe and Maroubra locations.

Key responsibilities

• Coordinate client services, rostering and workforce scheduling
• Manage client intake, onboarding, care plans and service agreements
• Support and supervise social support workers
• Liaise with clients, families and carers to make sure services meet individual needs
• Respond to service changes, enquiries and day-to-day operational priorities
• Maintain accurate client records and documentation
• Contribute to service improvements and continuous quality outcomes

Required profile

• Experience in aged care, community care, social support services or a similar environment
• Experience supporting, leading and supervising frontline staff
• Experience in goal setting and service planning
• Ability to manage multiple priorities in a fast-paced environment and adapt quickly when priorities change
