Registered Nurse - Transition to Residential Aged Care Unit

About the role

Hunter New England Health is recruiting a Registered Nurse for its 20-bed Transition to Residential Aged Care (TRAC) Unit. The unit supports older people who have recovered from an acute medical episode but cannot safely return home, helping them move into residential aged care with dignity and respect. The position is permanent part-time, 32 hours per week, with pay of $41.22-$57.88 per hour plus superannuation and salary packaging.

Key responsibilities

• Deliver person-centred nursing care that meets the individual needs of older patients
• Provide compassionate palliative care and emotional support to patients, families and carers
• Work with the multidisciplinary team to develop care plans and achieve positive patient outcomes
• Support patients and families through the transition to residential aged care
• Apply evidence-based nursing practice and contribute to continuous quality improvement

Required profile

• Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (AHPRA)
• Genuine interest in aged care and compassionate, person-centred care
• Strong communication and interpersonal skills, able to support patients, families and carers through challenging transitions
• Ability to work collaboratively within a multidisciplinary team
• Commitment to evidence-based practice, quality improvement and ongoing professional development
• Australian citizenship, Australian permanent residency, or a New Zealand passport; visa sponsorship is not available
