Practice Nurse – Chronic Condition Management

About the role

We are looking for a registered or enrolled nurse to join our Southport Medical Centre. In this role you will support general practitioners and patients by managing chronic conditions and promoting preventative health care within a collaborative, patient‑focused environment.

Key responsibilities

• Prepare Care Plans and Team Care Arrangements for patients.
• Assist patients in managing chronic conditions and adopting healthier behaviours.
• Conduct health assessments and promote preventative care initiatives.
• Collaborate with GPs and multidisciplinary teams to deliver coordinated care.
• Handle administrative tasks related to Chronic Condition Management (CCM) appointments.

Required profile

• AHPRA‑registered Enrolled or Registered Nurse.
• Prior experience in Chronic Condition Management is preferred.
• Strong communication and organisational abilities.
• Friendly, proactive, and patient‑focused attitude.
