About the role

The Nurse Navigator – Emergency Frequent Admissions is a registered nurse role focused on delivering safe, efficient, and coordinated care for patients with complex needs who frequently present to emergency services.

Key responsibilities

• Reduce service fragmentation by linking patients to appropriate community and hospital services.
• Partner with families and carers to enhance understanding of conditions and support self‑management.
• Coordinate multidisciplinary care plans to smooth the patient journey and improve outcomes.

Required profile

• Advanced clinical, evidence‑based nursing knowledge and recent practice experience.
• Leadership ability to foster positive team culture and multidisciplinary collaboration.
• Proven skill in therapeutic relationship building, health education, advocacy and culturally competent care.
• Experience applying quality‑improvement frameworks and contributing to nursing research.

Required skills

• Current registration with the Australian Health Practitioner Regulation Agency (Ahpra).
• Vaccination status compliant with Health Employment Directive NO.01/16.
