Wellness and Lifestyle Coordinator

About the role

Southern Cross Care is hiring a Wellness and Lifestyle Coordinator in Mount Barker, South Australia. The role delivers services that help residents take part in meaningful activities, improve their health literacy and stay independent. It involves connecting residents with the community, leading staff and volunteers and running proactive wellness strategies. The position is permanent part-time, 70 hours per fortnight, Monday to Friday between 08:30 and 16:00.

Key responsibilities

• Health promotion: carry out holistic assessments of residents' goals and preferences and design personalised wellness pathways for those at risk of decline.
• Person-centred care: create an inclusive environment, encourage participation, listen to feedback and support residents in making informed life choices.
• Governance and quality: ensure compliance with Aged Care Quality Standards, run continuous improvement initiatives and mentor staff, students and volunteers.

Required profile

• Diploma or equivalent in Health Promotion, Social Work, Dementia Care, Health Coaching or Leisure and Lifestyle.
• Proven experience conducting person-centred assessments (for example QUALID, WHOQOL-BREF) and evaluating program outcomes.
• Current driver's licence and Senior First Aid and CPR certificates are desirable.

Required skills

• Strong digital literacy, including Google Workspace.
• Experience with iCare, Person Centred Software (PCS) or similar aged care management software is advantageous.
• Good communication, time management and problem-solving skills, especially around resident risk and independence.
• Knowledge of current best practice in aged care, dementia care requirements and values-based goal setting.
