Social Worker - Subacute Ward

About the role

Albury Wodonga Health is recruiting a Social Worker for its Subacute Services Team in Wodonga. The Social Worker works with medical, nursing and allied health professionals in a multidisciplinary environment to deliver person-centred care, helping patients and their families cope with illness, injury, disability and changes in health circumstances. The role is full-time and permanent.

Key responsibilities

• Deliver high-quality social work services as part of the rehabilitation team, including psychosocial assessment, therapeutic intervention, advocacy and discharge planning.
• Support patients and families to adjust to illness, injury, disability and changes in functioning, building resilience and enhancing wellbeing.
• Contribute to safe, effective, person-centred care pathways into, within and out of the health service.
• Work in a dynamic, collaborative team with flexibility to balance work and life commitments.

Required profile

• Bachelor or Master of Social Work, with a minimum of 3 years' experience.
• Registration or eligibility for full registration with the Australian Association of Social Work (AASW).
• Clinical knowledge and expertise in social work relevant to the hospital setting.
• Understanding and applied knowledge of NDIS.
• Proven ability to relate and communicate with internal and external stakeholders from different professional backgrounds.
• Current permanent driver's licence, as cross-border travel may be required.

Required skills

• Demonstrated skills in data collection and data analytics.
• Availability to work weekend shifts and the on-call roster.
