Nurse Practitioner - Community Palliative Care

About the role

Albury Wodonga Health is recruiting a Palliative Care Nurse Practitioner for its Wodonga Community Palliative Care service. The role provides clinical leadership and mentoring and specialist palliative care consultation to improve the quality of life of people with life-limiting illnesses. It is a part-time permanent position of 24 hours per week, working within a multidisciplinary team of Clinical Nurse Specialists, Occupational Therapists, Social Workers, Grief and Bereavement workers and a Volunteer Coordinator.

Key responsibilities

• Provide clinical leadership and mentoring within the community palliative care service.
• Undertake specialist palliative care clinical consultation.
• Support the holistic needs of patients and their families in the community setting to alleviate suffering.
• Contribute to quality improvement and professional growth.

Required profile

• Current AHPRA registration and endorsement as a Nurse Practitioner of Palliative Care.
• Demonstrated ability to work autonomously and collaboratively in an advanced and extended clinical role.
• Ability to communicate effectively with a range of health care professionals.
• Ability to engage with and promote reflective practice across the nursing workforce.
• Ability to apply and promote evidence-informed practice at point of care.
