Service Coordinator - Support at Home (Aged Care)

About the role

Care Connect, a not-for-profit support provider, is hiring a Service Coordinator for its Support at Home team in Bella Vista, NSW. This is a full-time, maximum-term contract running until October 2027, with hybrid working (3 days from home). The coordinator provides high-quality administrative support and resolves client requests within agreed service levels, working closely with clients, service providers and internal teams.

Key responsibilities

• Act as the first point of contact for clients, helping them access services and navigate care options
• Promote the Care Connect proposition by coordinating between clients, client advisors and service providers
• Deliver outstanding customer service in every interaction
• Build strong client relationships and tailor solutions to individual needs
• Work with care managers and service providers to achieve the best outcomes

Required profile

• Experience in customer service, administration or coordination
• Advantageous: previous work in a high-volume inbound phone environment
• Advantageous: background in CHSP, health or community services
• Qualification in Customer Service, Business Administration or equivalent, or at least 3 years' experience in a high-volume inbound contact role
• Full Australian working rights, current Police Check and Working With Children Check (police checks arranged for successful candidates)

Required skills

• Strong communication, organisation and time management skills
• Proficiency with Microsoft Office and comfort learning new systems
