RN Triage - Virtual Care

About the role

Healthcare Australia (HCA) is recruiting a Virtual Triage Nurse (RN Triage) in Melbourne. The nurse conducts clinical assessments via telehealth, prioritises patients by urgency, risk and presenting needs, coordinates care pathways and directs patients to the right service. The role sits within HCA's 24/7 Virtual Care Centre, which offers remote healthcare to people in the onshore Immigration Detention network.

Key responsibilities

• Conduct virtual clinical assessments, prioritising patients by urgency, risk factors and presenting symptoms
• Obtain and accurately document patient histories, clinical information and care plans in electronic health records
• Coordinate referrals, diagnostics and care pathways with GPs, Nurse Practitioners, specialists and allied health providers
• Provide patient education and follow-up to support chronic disease management and self-management
• Work within a multidisciplinary virtual care team and contribute to quality improvement and safe, compliant practice

Required profile

• Bachelor of Nursing (or equivalent) and current unrestricted AHPRA registration
• Minimum 5 years' full-time experience as a Registered Nurse in Primary Health
• Experience managing complex and chronic health conditions in a primary care setting
• Experience in immigration detention or similar environments highly regarded
• Highly autonomous, thrives in fast-paced virtual environments, with strong clinical judgement, communication and interpersonal skills
