Registered Nurse – Hospital in the Home (Sydney)

About the role

The Sydney Local Health District is seeking a Registered Nurse to deliver Hospital in the Home (HITH) services from Concord Repatriation General Hospital. You will provide person‑centred, evidence‑informed nursing care to patients in their homes or community clinics, supporting recovery outside the acute hospital setting.

Key responsibilities

• Assess, plan, implement and evaluate nursing care for HITH patients in their homes or clinic environments.
• Collaborate with multidisciplinary teams to ensure safe, coordinated care.
• Document clinical observations, interventions and outcomes in accordance with NSW Health policies.
• Provide education and support to patients, families and carers regarding home‑based treatment plans.
• Maintain compliance with infection control, medication safety and occupational health standards.

Required profile

• Current registration with the Australian Health Practitioners Regulation Agency (AHPRA).
• Demonstrated understanding of evidence‑informed practice and critical thinking.
• Valid NSW driver’s licence (P1 or P2 acceptable).
• Commitment to delivering high‑quality, patient‑focused care.
