Registered Nurse - Community Nursing (NDIS)

About the role

Horizon Living Group, an NDIS-registered provider of community nursing and disability support services in Perth, Western Australia, is hiring a Registered Nurse for an on-site role. The nurse provides high-quality community nursing care and disability support to NDIS participants, working closely with support coordinators, allied health professionals and hospitals.

Key responsibilities

• Conduct nursing assessments, administer medications, manage wound care and other clinical procedures, and monitor participants' health status
• Develop and implement care plans and document clinical notes accurately
• Collaborate with support coordinators, therapists and other health professionals to ensure coordinated care
• Educate participants and families on health management and promote independence in daily living
• Respond promptly to changes in clinical needs while upholding NDIS standards, organisational policies and clinical best practice

Required profile

• Current AHPRA registration as a Registered Nurse and eligibility to work in Australia
• Demonstrated clinical skills in community nursing, including assessments, medication administration, wound care and health monitoring
• Experience with NDIS participants, people with disability, or in aged and community care settings
• Valid driver's licence and reliable transport for community-based work (if required by the role)
• Current police clearance, NDIS Worker Screening Check and relevant vaccinations

Required skills

• Strong communication and relationship-building skills with participants, families and multidisciplinary teams
• Ability to develop and follow person-centred care plans and maintain accurate clinical records
• Knowledge of NDIS practice standards, safeguarding and relevant legislation, or willingness to learn
• Ability to work autonomously, manage time and adapt to changing participant needs
