Home Care Coordinator

About the role

Southern Cross Care is hiring a Home Care Coordinator in Forest Hill, Victoria, for a permanent full-time position of 76 hours per fortnight, Monday to Friday 08:00 to 16:06. The coordinator supports person-centred services for clients within the Home Care portfolio, ensuring services are delivered according to each client's assessed needs, service plan and budget, and acts as the main point of contact for clients to resolve and escalate issues.

Key responsibilities

• Client onboarding, reviews and assessments, and development of client budgets
• Set up client services in a timely way
• Act as the primary contact for clients, Home Care Assistants and domestic staff in the region
• Advocate for clients so they can make informed decisions
• Handle service coordination tasks such as change requests and equipment purchases
• Complete administrative tasks linked to onboarding and coordinate vendors for the region

Required profile

• Certificate III in Individual Support (Ageing), Aged Care or Disability, or another related qualification
• First Aid and CPR certification
• Current Class Car driver's licence and own registered, insured car
• Demonstrated client support experience and experience with culturally and linguistically diverse clients, including Aboriginal and Torres Strait Islander peoples
• Basic knowledge of Aged Care Standards and in-home aged care program guidelines and funding
• Desirable: Diploma of Nursing with current AHPRA registration as an Enrolled Nurse
