Care Partner - Support at Home

About the role

A not-for-profit aged care provider in Brisbane offers in-home services including Support at Home, CHSP, respite, transport and aged accommodation. It is seeking two experienced Care Partners to build trusting relationships with clients and their families and deliver compliant, person-centred care management that promotes independence, wellness and reablement. The role is 9-5, office-based, with a requirement to drive to client homes, and reports directly to the CFO.

Key responsibilities

• Conduct holistic assessments, care planning and reviews in clients' homes
• Manage new client intake, onboarding and service agreements
• Develop and maintain care plans, budgets and documentation
• Roster best-matched staff and coordinate internal teams and external providers
• Monitor budgets and compliance in line with the Aged Care Quality Standards

Required profile

• An aged care qualification with demonstrated experience in a similar home care role
• Current First Aid and CPR, plus a National Police Check
• A valid Australian driver's licence and full Australian work rights
• Strong client relationship-building, communication and stakeholder skills

Required skills

• Proficiency in the My Aged Care Portal and care databases
• MS Office

What we offer

• Salary of $85,000 - $95,000 per year plus superannuation, negotiable based on experience
• Not-for-profit salary packaging (PBI)
