CHSP Service & Care Coordinator

About the role

A not-for-profit aged care provider serving Brisbane and South-East Queensland is looking for a CHSP Service & Care Coordinator in Spring Hill. The role coordinates services for Commonwealth Home Support Programme (CHSP) clients, from initial intake and onboarding through to ongoing service delivery, working closely with the rostering team and care workers. It combines client service coordination, administration and communication.

Key responsibilities

• Coordinate CHSP services in line with program guidelines and organisational policies
• Manage client intake and onboarding, including preparing and completing sign-up documentation
• Explain CHSP and Support at Home services clearly to clients and their families
• Coordinate new services and service changes with the rostering team
• Communicate service requirements to care workers
• Monitor changing client needs, support service reviews and assist with referrals for reassessment
• Maintain accurate client records, service notes and compliance documentation
• Assist with reporting, quality activities and general administration

Required profile

• Experience in aged care or community services, ideally within CHSP, Home Care Packages or Support at Home
• Understanding of service coordination, client onboarding and basic scheduling
• Strong organisational and communication skills, with attention to documentation accuracy
• Confidence explaining aged care programs to clients and families
• Valid driver's licence, full Australian work rights and a National Police Check (or willingness to obtain one)
• Nice to have: experience with rostering teams and care workers, familiarity with My Aged Care and CHSP guidelines, and previous coordinator or intake officer experience

What we offer

• Salary of $75,000 - $85,000 per year plus superannuation, negotiable based on experience
