Home Care Coordinator (Care Manager/Partner)

About the role

Healthcare Australia is looking for a people-focused Home Care Coordinator (titled Care Manager/Partner) to support clients across the Adelaide area. The role is client-facing and involves building strong relationships with clients and their families, coordinating services that meet individual needs and acting as a trusted point of contact throughout their home care journey. It combines time spent in the office with visits to clients' homes.

Key responsibilities

• Monitor service delivery through regular communication, care plan reviews and client feedback to ensure satisfaction and high standards.
• Collaborate with Registered Nurses and Allied Health professionals to coordinate care for clients with complex needs.
• Work closely with internal teams (Support Worker Service Delivery Lead, Recruitment and Rostering) so services are delivered efficiently and meet client expectations.
• Contribute to workforce planning by helping recruit and match suitable care workers.

Required profile

• Experience in home care, aged care, community services, healthcare, care coordination or case management.
• Enjoys working with people, building trusted relationships and delivering excellent customer experiences.
• Knowledge of Home Care or Support at Home services is highly regarded; broader healthcare and community services backgrounds are also considered.
