General Practitioner – Community Health

About the role

Cohealth is seeking compassionate General Practitioners to join its medical teams across Melbourne's Northern and Western suburbs. You will deliver comprehensive primary care within a multidisciplinary environment, collaborating with allied health professionals to address complex health and social needs.

Key responsibilities

• Provide high‑quality medical consultations and ongoing patient management.
• Work alongside GPs, nurses, pharmacists, social workers and care coordinators to develop and implement innovative multidisciplinary care models.
• Contribute ideas to improve care coordination, patient outcomes and service delivery.

Required profile

• RACGP‑registered General Practitioner with a current Victorian Medical Practitioners Board licence.
• Excellent communication and interpersonal skills, with a commitment to holistic, patient‑centred care.
• Alignment with cohealth’s values of health equity, social justice and human rights.

Required skills

• Ability to work effectively in diverse cultural contexts.
• Interest in collaboration across disciplines and innovation in care models.
