Enterprise Transformation Specialist - Adoption

About the role

This role is responsible for planning, communicating, coordinating and ensuring adoption of transformation initiatives across HammondCare. Working within the Transformation Office, you will support leaders and teams to prepare for, implement and embed new ways of working.

Key responsibilities

• Support leaders and stakeholders to understand, prepare for and lead transformation within their teams.
• Assess change impacts on people, processes and systems and ensure appropriate support is in place.
• Deliver clear, targeted communications to build understanding and confidence.
• Coordinate activities that help teams transition to new processes, systems and ways of working.
• Measure adoption outcomes, identify barriers and work with leaders to address issues.
• Maintain an organisation‑wide view of transformation activity to identify cumulative impacts and risks.
• Capture lessons learned to continuously improve transformation approaches.

Required profile

• Commitment to HammondCare’s Christian mission.
• At least 10 years’ experience supporting organisational transformation initiatives.
• Ability to assess change impacts across people, processes, systems and ways of working.
• Strong stakeholder engagement, communication and facilitation skills.
• Experience supporting business readiness and implementation activities.

• Experience developing and coordinating change, communication and stakeholder engagement activities.
• Ability to identify and manage readiness gaps, stakeholder concerns and adoption risks.
• Strong written and verbal communication skills, translating complex change into clear messages.
