Dementia Advisor – Residential Aged Care

About the role

A rewarding full‑time position for a passionate dementia professional to join HammondCare’s residential aged care team in Waratah. The Dementia Advisor provides expert advisory services, coaching, and case management to improve the lives of residents living with dementia.

Key responsibilities

• Implement HammondCare’s Philosophy of Care and case‑management framework.
• Provide on‑the‑ground dementia‑specific advisory services and participate in clinical review meetings.
• Coach and mentor staff, building capacity to manage dementia‑related situations and escalations.
• Plan and lead practice development initiatives, contributing to evidence‑based care practices.
• Evaluate, monitor and report on projects using HammondCare processes.
• Participate in behaviour‑management planning and related projects.

Required profile

• Minimum 3 years experience working with people living with dementia.
• Registered Nurse or Allied Health qualification (essential).
• Additional qualification in Training and Assessment (highly desirable).
• Strong communication, negotiation and relationship‑building abilities.
• Ability to work independently and as part of a multidisciplinary team.

Required skills

• Microsoft Office proficiency.
