Customer Care Manager - Home Care

About the role

Healthcare Australia is seeking a proactive Customer Care Manager to support home‑care clients across Melbourne’s South‑Eastern or Northern suburbs. You will be the primary trusted point of contact, delivering personalised services and guiding clients through their home‑care journey.

Key responsibilities

• Meet clients in their homes to assess needs and develop tailored care plans.
• Coordinate services with internal teams to ensure high‑quality delivery.
• Maintain accurate documentation and compliance with regulatory requirements.
• Build strong relationships with clients, families and support networks.

Required profile

• Experience in home care, aged care, community services, healthcare, care coordination or case management.
• Excellent communication, organisational and multitasking abilities.
• Customer‑first mindset with a passion for helping older Australians remain independent.
