Customer Care Coordinator - Home Care, Brisbane Northside

About the role

HCA's home care services help older Australians stay independent in their own homes, with personalised care plans developed by in-home care specialists. This full-time Customer Care Coordinator position covers the home care caseload in Brisbane Northside and acts as the main point of contact for new and existing clients, guiding them through their aged care journey.

Key responsibilities

• Act as the main point of contact for clients, delivering exceptional customer service and tailored home care solutions.
• Build and maintain relationships with clients and their families to understand their goals, preferences and individual needs.
• Conduct assessments and develop personalised care plans that support client independence and wellbeing.
• Monitor service delivery through regular communication, care plan reviews and client feedback.
• Collaborate with Registered Nurses and Allied Health professionals on care coordination for clients with complex needs.
• Work with internal teams such as Support Worker Service Delivery Lead, Recruitment and Rostering to deliver services efficiently.
• Contribute to workforce planning by helping recruit and match care workers.

Required profile

• Strong communication and relationship-building skills with a customer-first approach.
• Proactive, results-driven, able to meet business targets and deadlines.
• Solid understanding of the Home Care and broader health services environment, and experience in a customer-focused service business.
• Ability to work independently and as part of a wider team.
• Current National Police Check (ACIC accredited) stating suitability to work in aged care or with vulnerable persons.
• Full driver's licence and own registered, insured vehicle.

Required skills

• Technology and systems competency with MS Office and CRM systems.
