Coordinator – Home & Community

About the role

The Community Services Coordinator will deliver person‑centred home and community support for clients of St Jude’s Disability Services. Based in the new West Perth office, you will build lasting relationships with clients, families and stakeholders while overseeing the team that provides direct services.

Key responsibilities

• Work closely with clients to ensure NDIS funds and services are used effectively.
• Verify that support plans are followed and that high‑quality progress notes are maintained.
• Manage staffing levels, supervise support workers and conduct performance appraisals.
• Monitor budgets, utilise allocated funds responsibly and meet financial targets.
• Ensure compliance with NDIS standards, manage risks and maintain service quality.

Required profile

• Proven experience delivering quality, person‑centred services in community or disability programs.
• Experience supporting participants with complex mental health issues or challenging behaviours.
• Strong understanding of the NDIS funding model and reporting requirements.
• Proactive self‑starter with a focus on continuous improvement.
• Willingness to obtain NDIS screening clearance, Working With Children Check, WA driver’s licence and reliable vehicle.
