Support Coordinator (NDIS)

About the role

MyIntegra, an NDIS-registered provider and part of the APM Group, is looking for experienced Support Coordinators in Brisbane. You will work alongside NDIS participants to help them navigate their plans, connect with the right supports, overcome barriers and build greater choice, confidence and independence.

Key responsibilities

• Navigate NDIS participant plans and connect them with appropriate supports
• Work through barriers to help participants exercise choice and build independence
• Build trusted relationships with participants, families, providers and other stakeholders
• Manage a caseload, prioritising effectively and following through on tasks

Required profile

• Demonstrated Support Coordination experience and a strong understanding of the NDIS; experience with more complex needs is welcomed
• A relevant qualification (Cert III or above) in disability, mental health, allied health, community services or another human services discipline is welcomed alongside practical experience
• Strong knowledge of the local service landscape and networks
• Resourceful problem solver who can handle competing priorities and multiple stakeholders
