About the role
Amplar Health is expanding its Support at Home service, delivering clinically‑led, community‑based care to older Australians. As a Community Registered Nurse you will work autonomously to provide person‑centred care, helping clients stay independent and connected in their own homes.
Key responsibilities
- Collaborate with Clinical Care Partners, allied health and nursing professionals to deliver coordinated, person‑centred care.
- Assess client health, develop care plans and support early intervention and re‑ablement strategies.
- Influence health outcomes across the care journey by integrating with Amplar’s primary‑care network.
Required profile
- Current unrestricted AHPRA registration as a Registered Nurse.
- Minimum two years’ experience in aged‑care and community settings.
- Residence in or near North Gold Coast or Central Queensland.
- Strong communication and relationship‑building abilities.
- Passion for delivering outstanding patient experiences.
