Clinician - Case Management (Mental Health)

About the role

Western Health's Mental Health and Wellbeing Service is recruiting a Community Mental Health Case Management clinician for the Maribyrnong Community Team/Saltwater Clinic. The role suits an experienced, recovery-focused clinician with an allocated caseload of consumers with complex mental health needs. Care is delivered through assessment, care planning, intervention and coordination of services, working with families, carers, the multidisciplinary team and external providers.

Key responsibilities

• Deliver recovery-oriented, person-centred care through assessment, care planning, intervention and service coordination.
• Work with families, carers, the multidisciplinary team and external providers to support safety, wellbeing and community participation.
• Maintain timely documentation, monitor clinical risk and apply the Mental Health and Wellbeing Act 2022 in daily practice.
• Take responsibility for an allocated caseload, working autonomously within scope of practice and escalating concerns when needed.
• Communicate clearly and respectfully with consumers, families, carers and service partners, building therapeutic relationships.

Required profile

• Registered Psychiatric Nurse with AHPRA registration.
• Experience working in public mental health.
• Sound clinical judgement and risk assessment skills.
• Current Victorian driver licence.
• Confident in fast-paced community settings and able to respond to acute mental health distress.
