Clinical Pharmacist – Hospital in the Home & Residential Inreach (Part‑time)

About the role

This part‑time, fixed‑term position offers an experienced Clinical Pharmacist the opportunity to deliver specialist pharmacy care to patients in their homes and residential care facilities. You will work within the Hospital in the Home (HITH) and Residential Inreach (RIR) services of a leading Australian health provider, collaborating with multidisciplinary teams to ensure safe and effective medication use.

Key responsibilities

• Provide clinical pharmacy services to HITH and Residential Inreach patients in their homes and residential care facilities.
• Obtain comprehensive medication histories and perform medication reconciliation.
• Review medication therapy and make recommendations to optimise safety and efficacy.
• Deliver medication counselling and education to patients, families and carers.
• Collaborate with medical, nursing, pharmacy and other health professionals, including community pharmacies and GPs.
• Support safe transitions of care between hospital, home and residential settings and maintain accurate clinical documentation.

Required profile

• A recognised pharmacy qualification and current AHPRA registration.
• Minimum three years’ experience in a hospital pharmacy setting.
• Strong clinical judgement and excellent written and verbal communication skills.
• Ability to work independently and as part of a multidisciplinary team.
• Current satisfactory National Police Certificate or willingness to obtain one.
