Chronic Condition Management Nurse

About the role

CQ Medicentre South is looking for an experienced Registered or Enrolled Nurse to join its established practice in Rockhampton. The role focuses on chronic condition management and preventative healthcare, supporting patients with chronic and complex conditions through care planning, education and ongoing management. It suits someone who enjoys building long-term patient relationships.

Key responsibilities

• Coordinate chronic condition management plans
• Conduct care plan reviews and complete health assessments
• Deliver patient education and health coaching
• Manage recalls and reminders
• Support preventative healthcare initiatives
• Work with GPs and allied health providers
• Monitor patient outcomes and follow-up
• Maintain accurate clinical documentation

Required profile

• AHPRA registration as a Registered or Enrolled Nurse
• Current CPR certification
• Experience in general practice nursing
• Demonstrated experience with, or ability to learn, chronic condition management plans, care plan reviews, health assessments and patient follow-up programs
• Excellent organisational and communication skills
• Full Australian working rights

Required skills

• Experience using best practice software is highly regarded
