Care Partner - Home Care Team

About the role

HammondCare is looking for a full-time Care Partner on a 6-month contract ending April 2027 to join its Home Care Team based in Hammondville. The Care Partner delivers and develops tailored care plans for each client and oversees the ongoing assessment of clients' needs and preferences.

Key responsibilities

• Assist in delivering services tailored to individual clients, within budget and service expectations, including ongoing client assessment and care plan development with key stakeholders.
• Manage the care of existing clients and client-related KPIs.
• Keep care plans up to date so clients receive the right type and amount of care matching their needs, approvals and funding.
• Deliver services professionally and competently, ensuring staff understand each client's needs and preferences.

Required profile

• Certificate IV in Aged Care and at least 2 years of relevant industry experience.
• Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and the Commonwealth Home Support Programme.
• Experience working effectively in a multi-site environment.
• Current driver's licence and willingness to travel between clients, sites and services (a salary package can include a motor vehicle on request).

Required skills

• Strong administrative and computer skills.
