Care Coordinator

About the role

Benetas Community Health and Care (CHC) provides in-home aged care services to older clients across Victoria, including CHSP, Support at Home, Veterans Home Care, respite and private in-home care. Its Care Coordinators help clients make the best use of these services so they can live at home longer. The position is based in Blackburn, Victoria.

Key responsibilities

• Complete client intake and onboarding processes
• Coordinate services in partnership with clients, families, carers and the Benetas team
• Help clients understand their budgets and maximise available resources
• Promote independence, choice and informed decision-making through clear information
• Interpret and apply policies, procedures and aged care legislation
• Assist with developing and reviewing holistic client support plans
• Advocate for clients and liaise with external service providers where needed
• Manage a high volume of calls while keeping accurate client records
• Support compliance with the Aged Care Act and service standards

Required profile

• Genuine passion for supporting older people to live well
• Strong communication and problem-solving skills
• Demonstrated administration skills and organisational methods
• Ability to work independently and in a team
• Respect for diversity and ability to build trust with clients and families
• NDIS Worker Screening Clearance required for eligibility

Required skills

• Intermediate computer skills and ability to navigate systems
• Administration and record-keeping

What we offer

• $39.69 per hour plus 12% superannuation
• Salary packaging up to $18,300 per year
