About the role
Join the SURe (Substance Use Recovery) team to support individuals facing alcohol and other drug challenges, mental health concerns, and complex life circumstances. You will work in a compassionate, multidisciplinary environment to help clients navigate treatment, build resilience, and achieve recovery goals.
Key responsibilities
- Provide recovery‑focused support through engagement, assessment, care coordination and case management.
- Develop personalised recovery plans that reflect individual strengths, goals and needs.
- Coordinate access to treatment and support services across health, housing, family support and community sectors.
- Facilitate case conferences and care planning to support integrated and collaborative care.
- Build strong stakeholder relationships with GPs, mental health services, housing providers and other community partners.
- Deliver harm‑reduction education and recovery interventions using trauma‑informed and dual‑diagnosis principles.
- Monitor client progress and outcomes to support continuous improvement and quality service delivery.
Required profile
- Relevant qualification in Youth Work, Social Work, Alcohol and Other Drugs, Community Services (minimum AOD Skill Set).
- Experience working within AOD, mental health, community services or related sectors.
- Strong case management, care coordination and assessment skills.
- Excellent communication and stakeholder engagement abilities.
- Commitment to cultural safety, inclusion and respectful practice.
Required skills
- Ability to apply trauma‑informed, recovery‑oriented and person‑centred approaches.
- Strong documentation, report writing and administrative skills.
- Ability to manage competing priorities and work collaboratively within a multidisciplinary team.
