Behaviour Support Practitioner (Toowoomba)

About the role

Outcomes Connect Australia (OCA) is recruiting Behaviour Support Practitioners in Toowoomba, experienced or entry-level, to help people living with disability improve their quality of life, reduce behaviours of concern and promote their rights. You will work with clients (NDIS participants), their families and carers, alongside a team of practitioners, a team leader and clinical advisors, using evidence-based practice. You must live in Toowoomba.

Key responsibilities

• Help clients, families and carers identify needs through assessment and person-centred planning
• Set meaningful goals and practical strategies using The Positive Behaviour Support Capability Framework, eliminating restrictive practices in the home and community
• Develop, deliver and evaluate evidence-based support plans in a complex support space
• Make sure individual plans reflect clients' goals, passions and strengths
• Build the knowledge, capacity and strengths of family, carers and friends who support clients

Required profile

• Current suitability (registration) with the NDIS Commission as a Behaviour Support Practitioner is preferred but not essential
• Considerable practical experience supporting people with complex and challenging behaviours
• Degree-level qualification relevant to supporting people with behaviours of concern highly preferred
• Current driver's licence and own reliable vehicle
