Aged Care Clinical Assessor

About the role

As a Clinical Aged Care Assessor you will conduct comprehensive assessments for older adults across the Far North Queensland region, linking them with services that support independent living and high quality of life. Daily travel throughout the service area is essential to meet client and business needs.

Key responsibilities

• Perform high‑quality Aged Care assessments to identify client needs.
• Facilitate referrals to essential services that promote independence.
• Promote retention and maximisation of functional capability in daily living.
• Engage with families, carers and other stakeholders as key partners.
• Ensure compliance with Department of Health contractual requirements.

Required profile

• Minimum 1 year experience working with older Australians, including assessments or service delivery.
• Tertiary qualification in a health‑related discipline and registration with a relevant professional body (e.g., AHPRA).
• Current Australian driver’s licence, comprehensively insured vehicle and ability to travel within the service area.

Required skills

• Proficiency in Microsoft Word, Excel and Outlook.
• Experience using client management systems.
