About the role
APM Group is seeking qualified health professionals to join its network of Clinical Aged Care Assessors. The role involves conducting in‑home and residential assessments across your local service area, offering flexible scheduling to suit your lifestyle.
Key responsibilities
- Perform high‑quality Aged Care assessments to identify the needs of older adults.
- Facilitate referrals to services that support independent living and promote functional capability.
- Engage with families, carers and other stakeholders as key partners.
- Ensure all assessments comply with Department of Health contractual requirements.
Required profile
- At least 1 year experience working with older Australians in assessment or service delivery.
- Tertiary health‑related qualification and registration with a relevant professional body (e.g., AHPRA).
- Current Australian driver’s licence, insured vehicle and ability to travel daily within your service area.
Required skills
- Proficiency in Microsoft Word, Excel and Outlook.
- Experience using client‑management systems.
