Veteran Care Nurse Coordinator (RN)

About the role

VeteranCann is seeking a compassionate Registered Nurse to work part‑time as a Veteran Care Nurse Coordinator. The role blends clinical nursing, care coordination and telehealth to support current and former service members across Australia.

Key responsibilities

• Coordinate and manage ongoing care for veterans and other patients, ensuring continuity and regular wellbeing follow‑ups.
• Conduct telehealth pre‑screening consultations, patient intake assessments and comprehensive medical history reviews.
• Prepare patient files and clinical information for GP review, identifying clinical risks, medication interactions and escalation needs.
• Provide face‑to‑face nursing consultations and intake assessments for new clinic patients.
• Support chronic disease management, preventative health initiatives and veteran health reviews.
• Liaise with GPs, specialists, allied health providers, pharmacies, hospitals and community support services.
• Assist with hospital admission and discharge coordination, and build therapeutic relationships with veterans and families.
• Identify psychosocial and mental‑health needs, facilitate appropriate referrals and educate patients on treatment pathways.
• Maintain accurate clinical records in line with AHPRA, Medicare, DVA and privacy requirements.
• Participate in quality improvement initiatives, clinical audits and professional development activities.

Required profile

• Current unrestricted registration as a Registered Nurse (RN) with AHPRA.
• Experience in patient‑centred care, care coordination and telehealth delivery.
• Strong understanding of veteran health issues and ability to work collaboratively with multidisciplinary teams.
• Excellent communication and advocacy skills.
