Support Coordinator

About the role

MyIntegra, an NDIS-registered provider and part of the APM Group, is hiring a Support Coordinator in Darwin. You will help NDIS participants understand their plans, connect with the right supports and build greater choice, confidence and independence. Experience in support coordination is not required.

Key responsibilities

• Help NDIS participants understand their plans and connect with the right supports
• Build greater choice, confidence and independence for participants
• Manage a caseload and follow things through
• Build trusted relationships with people and understand what is available locally

Required profile

• Experience in disability, mental health, community services, allied health, case management or another relevant human services setting
• A relevant qualification is welcomed; willingness to undertake further study is valued if you do not have one
• Ability to organise your work, manage a caseload and take ownership
• Strong communication skills and knowledge of local community resources
